(You can save your progress, and a link will be emailed to you to return.) Once complete, a member of our team will review and approve your application. You will then be emailed a Deposit Instruction form to complete, which will enable you to fund your account.

    Deposit Slip: Many banks, even if they don’t provide pre-voided checks, offer direct deposit forms. These forms are like pre-filled direct deposit slips that you can provide.

    Bank Documentation: A letter or statement on bank letterhead with your account information could work.

    At least one beneficiary is required. Do not list joint account holders as beneficiaries, as they already have full rights to the account.
